LIABILITY WAIVER AND RELEASE OF CLAIMS

Livingston Ice Skating Association 3-on-3 Adult Hockey League 2027

PARTICIPANT INFORMATION

ASSUMPTION OF RISK AND RELEASE

I, the undersigned participant, acknowledge that I am voluntarily participating in the Livingston Ice Skating Association's 3-on-3 Adult Hockey League ("the League") at the Park County Recreational Rink. I understand and acknowledge that participation in ice hockey and related activities involves inherent risks and dangers including, but not limited to, the following: collisions with other players, the puck, boards, or ice surface; falls on the ice; equipment failure; cuts from skate blades; injuries from sticks or pucks; head injuries including concussions; sprains, strains, fractures, and other bodily injuries that may result in temporary or permanent disability or death.

I understand that these risks cannot be eliminated regardless of the care taken to avoid injuries, and I voluntarily and knowingly assume all such risks. I accept personal responsibility for any damages, liabilities, or injuries that may result from my participation in the League.

RELEASE AND WAIVER OF LIABILITY

In consideration of being permitted to participate in the League, I hereby release, waive, discharge, and covenant not to sue the Livingston Ice Skating Association, Park County, Park County Recreational Rink, and their respective officers, directors, employees, volunteers, agents, sponsors, and representatives (collectively "Released Parties") from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, or injury, including death, that may be sustained by me or my property while participating in the League or while on the premises of the Park County Recreational Rink.

MEDICAL TREATMENT AUTHORIZATION

I authorize the Released Parties to secure emergency medical treatment for me if necessary, and I agree to assume responsibility for all costs associated with such treatment. I acknowledge that I have adequate health insurance coverage or accept financial responsibility for any injuries sustained while participating in the League.

EQUIPMENT AND RULES

I certify that I am physically fit to participate in ice hockey activities and have no medical condition that would prevent my safe participation. I agree to wear appropriate protective equipment including helmet with face protection, gloves, shin guards, and other standard hockey safety equipment during all League activities. I agree to abide by all League rules and follow the directions of League officials and rink staff.

ACKNOWLEDGMENT OF UNDERSTANDING

I have read this waiver and release, fully understand its terms, and understand that I am giving up substantial legal rights including the right to sue. I acknowledge that I am signing this agreement freely and voluntarily and intend my signature to be a complete and unconditional release of all liability to the greatest extent allowed by law.
